What is this service?
Hospital to Home Support is a new service for carers in Mid Essex who are looking after someone who is under the care of Hospital from Home or Virtual Ward support.
The service consists of:
- A new Carer Support Adviser providing 1-2-1 support for carers
- New resources including a Hospital to Home Discharge Guide and Hospital to Home Checklist
- Significant Care Tool and training for carers
